I usually ship via insured EMS Speedpost courier. You can find postage rates here. Parcels typically weigh 600-900 grammes but pens with large boxes can weight more. Insurance cost is 3% of the declared value of the pen. Although EMS Speedpost is very safe and efficient, I still recommend insurance as the additional cost is not substantial.

As for payment, I prefer payment via bank wire (telegraphic transfer). However, as I understand that can be costly for the buyer, I also accept payment via PayPal for items that are below $250 in value. That's not a hard and fast rule, so contact me if you are interested in purchasing something.
